If you are injured at work, it is important that you complete the district's Employee Report of Incident form and submit it before the end of your work day. If your injury is such that you are unable to complete the report, your administrator or supervisor must do so by the end of the day.The district will designate medical providers to be used in the administration of workers' compensation claims and treatment.
